Our Service

What do we do ?

KCHT provide social care support services to new and existing residents with the aim of providing them with independence and choice.

Residents support

We support residents to live independently by maintaining a healthy lifestyle, integrating into the community and keeping their home in a safe and habitable condition. For example we assist residents to maximise their income by accessing benefits and grants, and also with safety and security in their home.

We also support residents to access community resources and build social contact and involvement.

General Support

KCHT offer residents support and advice based on an assessment of their needs. KCHT also help with accessing services such as health and social services and advising on any other issue that residents may wish to discuss.

Debt / Budgeting Advice

KCHT provide assistance with prioritising debts including rent arrears and guidance on dealing with creditors and other agencies.

Advocacy

KCHT will liaise with other people / organisations on resident's behalf, including organising access to professional help as well as specialist voluntary organisations and support providers.

How do we do it ?

The Support Officer will arrange a meeting to undertake a need assessment and draw up a support plan, which sets out how KCHT can assist with any identified problems. The Support Officer will assist for as long as it is agreed to.

Housing and Maintenance

The Support Service is separate to the housing service. Concerns regarding housing and maintenance or repairs should be raised with London & Quadrant. KCHT will help resolve any housing related problems, the Support Officer will act as an advocate/mediator on the resident's behalf as agreed with the resident.

Confidentiality

The Support Service is separate to the Housing Service and information provided to the Support Officer is confidential and kept separately from the Tenancy File.

The Support Officer will not become involved in any tenancy enforcement action [due to breach of tenancy] other than to act as an advocate/mediator on the resident's behalf.
